Relax, even if it's you're first time it's going to be easy, this new patient guide will help you.

  • 1st: call our Capital Hill office at 206-860-9090. Heather, our office manager, will patiently guide you through the process, gather the necessary information, and reserve an appointment for you.

  • Please click on "Health History Form" below. Then download the form, print them out and complete. You can now either fax them to 206-860-3699 or bring them with you on your initial visit. Thank you!

  • As a courtesy to our patients we will call and verify your insurance benefits for chiropractic, acupuncture, and massage. Please have your insurance card ready when you call the office.
